My 12v Battery discharges even when driving

My car was running fine but sometimes i would have check engine code saying that my hybrid battery is weak, i would delete the code and it will appear around 2 week later again.
Now i was out on vacation and the car sat for a week. When i got back i had check engine and battery light on. My 12v battery was dead so i changed a new one and bearly made it to a service (the 12v battery was discharged) to check the codes. Code was P1570. The car simply shuts off. I suppose it is running on the 12v battery that’s why it runs out.
What can be the problem, how can i fix it

I assume the red battery light is on.
The hybrid battery can deteriorate to the point that the DC-DC converter is disabled.
Try this:
Disconnect the 12V
Manually charge the 12V until full
Remove rear seat back.
Remove safety switch cover
Turn off safety switch.
Connect 12V.
Attempt to start. It may take several seconds with the key in the start position before the backup starter cranks.
If the engine starts, hold idle at 2000 rpm for 30 seconds. If the red battery light goes out, you can likely drive with the hybrid battery disabled, but you will only get charging between about 1600 and 3500rpm.

Yes. P1570 is Battery Module Individual Voltage Problem meaning the 11 voltages measured every 12 cells deviate substantially. This is typically indicative of multiple cell failures.
Proper grid charging MAY restore function. Replacement with a new or known healthy battery should resolve the issue.

I had the same prob a few months ago.. you need a new hybrid battery... If you want 45 MPG you'll have to get a brand new one from the dealer for $3500.. You can get a new high capacity chinese cell one for like $1650 but the MPG will only be high 30's
DO NOT get a 'used', 'reconditioned', or 're-manufactured' pack. Second gen civic hybrids are very finicky about quality of their hybrid batteries. All the cells have to be of the exact same capacity & quality, or you may only get as low as 30 MPG.
One thing you can do while driving it around without the hybrid battery tho is make sure to always keep the RPMs between 2300 & like 3500. I did this by always driving in L in neighborhoods, S in the city, and only in D when on the freeway. Also if you sitting at a light put it in N and step on the gas to hold the RPMs in the range.
That will keep the 12v battery charged... Its only when its above or below that that the 12v battery is being drained and not recharged...
You should get a little volt meter that plugs into the cigarette lighter, and keep it at or above 13v by keeping the RPMs up.
