Preview (a Mac application) does it very easily. All you do is drag the icon for the page(s) from one file (let's call it pages 1-10) onto the screen of another file (let's call it pages 11-20). The problem is eventually you need to drag a huge file (pages 1-240) onto another (pages 241-445). That's the PITA! Merging two pages with a third is easy. Try doing it with huge files and you will see the difficulty.

I used ConcatPDF to merge all the files into the 2007 HV Owners Manual. It is free, easy to configure and quite fast. Unfotunately it is only available as a PC Windows program.

I used PDF Split and Merge. It has a Mac beta.

Would these programs have 'coughed and wheezed' at merging hundreds of pages?
Would RAM matter? I'm at 1.25GB and wonder if 2-4 GB would have made this a snap.

I used a 900Mhz 512MB system running XP to merge 396 pages and it surprised me at how fast it ran. The only tedious part was arranging all the file names in the correct order prior to starting the merge.

Hint....I saved each set with the file name "15-22", etc. making ordering them, somewhat, easy.
I don't know what I did wrong, if anything, but the file size for the 2008 is significantly larger than the 2007 version.
All I did was open the PDF at Toyota's site, right click and Save As.. to download the file, then merge them.
The 08 might be a 50 pages longer, but my file was >41MB and the 07 is around 6MB.
That might account for my computer's asthma?

Out of curiosity, does merging the PDFs cause loss of the text search function? In a quick test, text search only worked on one small section of the 08 manual. Is this just me or can anyone else see this issue?

Ouch! Yeah, searching didn't find any results
Oh well, at least there is a table of contents in the front and an alphabetical index in the back.

If you still have the individual sections and they are searchable, maybe you could post a zip file of those for the people who are interested in searching the text. The standard Adboe PDF viewer can search multiple files in the same directory.
