It's a 08 correct? Any codes showing up?

One way to find out is eliminate the MC by replacing it since you said the codes points to nothing

Some of those codes could be from bad bleed. The 08s were a pain to get the bleed right.

How many miles? Sometimes eventual failure has happened due to stop and go use of the brake pedal..

How often would u need to bleed brakes ...not on pad changes ...right ?

Dealers were not ready for many of the new systems in hybrids. Some still are not. The 08 has many more pressure sensors in the main brake module ( under the driver's seat ) under floor that later models don't have. If that system has been opened and or improperly adjusted, the system can and usually does act up. There are very fussy procedures for the bleeding of hydraulics and calibration of the sensors. If either had been done in the past, a re-do by an experienced dealer may be in order. Either way getting the technical line folks to help should minimize just Easter Egg hunting for the problem. Before swapping expensive parts, have more than a guess on your side.
